 PLEASE SUBMIT THE ORDER FORM VIA REGULAR MAIL.  IT CAN BE FILLED-IN ONLINE AND PRINTED FOR MAILING.  

The Order Form is designed to be filled-in and "Submitted" online or printed and completed manually.  If you order Educational Materials, do not enter Shipping Charges.  

These files are Adobe Acrobat .pdf files.  You will need the Adobe Acrobat Reader to read or print the files.  To save the file to your computer, right-click the link and select "save target as..." (Internet Explorer) or "save link as..." (Netscape).

Even if you already have Adobe Acrobat Reader, you should make sure you have the latest version.
